  • Attestation: MOFA + Civil Surgeon/Hospital Superintendent
  • Must include:

    • Not any medication details
  • CNIC & passport/visa copies
  • MOFA QR verification
  • Must not be more than 3 months
  • For Govt Employee, original NOC with MOFA Attestation
  • For Medicines, must get prior permission from Health Regularity Authorities from UAE.
